As the Social Worker / Therapist, you will support youth within two programs:The Welcoming Center (TWC)andImmediate Stabilization Services (ISS). In this role, you will provide exceptional trauma-informed therapeutic services, implement crisis intervention, and coordinate care with caregivers, family members, and other community service providers. You will have the opportunity to participate in treatment reviews, utilize crisis communication, and provide services within the community.
ABOUT TH E WELCOMING CENTER & IMMEDIATE STABILIZATION SERVICES
The Welcoming Centeris a therapeutic, home-like setting that supports young people as they navigate the trauma of placement disruption.The Welcoming Centeris specifically designed to minimize the impact of this trauma by providing safety, care, comfort, and therapeutic supports for up to 24 hours while the best and most appropriate placement is coordinated.The Welcoming Centerserves young people in Santa Clara County who are removed from their home, returning from elopement, or otherwise experiencing a loss of placement. When they arrive atthe Welcoming Center, they are met by empathetic and well-trained adults that provide a calm, stable environment while their individualized needs are assessed and placement is secured.
Immediate Stabilization Services (ISS)is a community-based program thatsupports young people, their families, and/or resource parents in times of crisis and high need to reduce the likelihood of placement disruption. Prevention and stabilization services are available 7 days a week. ISSworks withyoung people who are dependents of Santa Clara County or at risk of child welfare involvement, including those served through hospital diversion. A family enrolled inISS will work with an assigned clinician, a facilitator,and a team of counselors to provide individualized interventions. TheISSteam collaborates with youth and caregivers to createtrauma informed care plans designed to increase safety and prevent placement disruption, so that youth and caregivers feel empowered and supported while in the program.
