Social Worker - Rapid Access Center - Northwell Health : Job Details

Shift Details: Monday - Friday, 9 AM - 5 PM. No weekends/holidays. Rapid Access Center: Mather Hospitals innovative Rapid Access Center is a point of entry for care for individuals age 18 and up who are experiencing a mental health or substance use disorder issue and do not know where to go for help. A clinical social worker and nurse practitioner performs an initial assessment and connects individuals seeking help with the appropriate services. Job Description: Provides individual, and/or group work services to patients and/or their families in various health care settings. Utilizes psychosocial assessment skills to deliver social work services. Job Responsibility: Provides culturally competent direct service to patients and their families using a variety of social work modalities in order to help individuals maximize their functioning.
