Overview
Under the direction of the Clinical Supervisor or designee, the Social Worker is responsible for providing social work services to assist in the resolution of social and emotional problems through appropriate psychosocial assessment and discharge planning for referred patients in an inpatient medical, surgical, or pediatric unit, or ambulatory care setting.
Responsibilities
Establish and maintain positive relationships with patients, visitors, and other employees. Interact professionally, courteously, and appropriately with all stakeholders, while promoting a positive public perception of Bronx-Lebanon Hospital Center.Carry an active caseload of patients/families as stipulated by departmental policies.Provide social work services to patients/families, including biopsychosocial/cultural assessment and treatment; crisis intervention, whether short or long term, utilizing modalities such as individual and family therapy.Collaborate with the interdisciplinary team in providing patient care.Provide reports, record documentation, and statistics as required.Serve as a liaison within the department, hospital, and to community agencies.May be delegated responsibility for in-service training and teaching social work students. Represent the social work department in job-related community meetings and conferences.Perform other activities as deemed appropriate to ensure services to patients and the effective functioning of the Social Work Program.Qualifications
Sequence of study or experience in clinical practice in accordance with current New York State standards for licensure.New York State Licensed Social Worker (Legally Required).Master's in Social Work (Required).Computer literacy (Required).#J-18808-Ljbffr
