We are seeking a Social Worker, also referred to as a Clinical Social Worker or Medical Social Worker. The Medical Social Worker will support patients and families by addressing emotional, social, and practical needs related to illness or injury. Medical Social Workers assist with discharge planning, crisis intervention, and connecting patients to community resources and services.
