Hospice Social Worker Make a Lasting Difference Pay up to $32 per hour (DOE) + mileage reimbursement The Heart of What We Do At Auburn Crest Hospice, we believe hospice care is about honoring every moment with dignity, compassion, and respect. As a Hospice Social Worker, youll be the emotional anchor for patients and familieshelping them navigate the practical, emotional, and spiritual challenges that come with life-limiting illness. Youll provide counseling, connect families to vital resources, advocate for their needs, and collaborate closely with our nurses, physicians, and spiritual care team. Every day, your work will make a direct impact on someones comfort, peace, and well-being. What Youll Do Provide psychosocial assessments and bereavement support for patients and families. Offer short-term crisis intervention and ongoing emotional support. Assist families in accessing resources like Medicaid, insurance benefits, and community programs. Advocate for patients needs while collaborating with the full care team. Maintain timely, accurate documentation of visits and referrals. Participate in bereavement programs, volunteer training, and community outreach. Qualifications Required: Bachelors degree in Social Work (CSWE-accredited) and active state Social Work license. Preferred: Masters degree in Social Work (MSW) and hospice or medical social work experience. Minimum 1 year in hospital, hospice, home health, mental health, or related setting.
