Social Worker - The University of Rochester : Job Details

The social worker provides clinical social work services to adult patients and their families in ambulatory clinics. Social work services provided include psychosocial assessments, information and referral, psychosocial treatment and other services required to optimize the patients' health care. The social worker collaborates with the health care team in the development and implementation of the care plan of the patients with whom the social worker is active. The social worker also provides consultation to the health care providers regarding patients' care needs and/or community resources. - - ESSENTIAL FUNCTIONS - - Performs psychosocial assessments of patients and families referred by clinic providers or identified through high risk screening.
