Social Worker - St. John's Episcopal Hospital - Episcopal Health Services : Job Details

Conducts daily intakes for patients discharged from Psych ED and/or Behavioral Health inpatient units Ensures all patients discharged from Psych ED are connected to care; Works in conjunction with hospital staff to facilitate transitions of care Provides brief follow-up intervention services until connections in the community are made Carries a caseload; Provides individual and group psychotherapy as dictated by the treatment plan Participates in interdisciplinary rounds and teams meetings Participates in hospital wide committees as indicated Participates in quality assurance and performance improvement activities as required Documents service provision as per applicable standards in the clinical EMR Other duties as assigned Requirements: Master s Degree from an accredited Master of Social Work Program Licensed Master Social Worker (LMSW)/ Licensed Clinical Social Worker (LCSW) preferred Experience working with psychiatric population, preferably with experience in psychiatric social wo...Social Worker, Worker, Behavioral Health, Healthcare, Patient
