Provides professional social work services to patients and their families. Collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients. Participates in formulating procedures and policies of the Division of Social Service. Participates in educational and research programs, including the orientation and training of new staff and students. Job Description - - ESSENTIAL FUNCTIONS - - Serves as intake therapist, performing diagnostic/intake evaluation and determination regarding appropriateness for level of care for assigned patients. Serves as primary therapist to assigned caseload as needed. This may be in the following modalities: individual, family and/or group therapy. Coordinates activities related to the comprehensive treatment and continuity of care planning for the patients assigned to his/her caseload.
