Social Worker - Westchester Medical Center Health : Job Details

Directly provides counseling, case management and other chemical dependency services in meeting the social emotional needs of the detox/rehab patients and their family members. Responsibilities: Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively. Provide evaluations and psychosocial assessments for patients in collaboration with the td team. Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients. Prepares and keeps a current case record for each assigned patient according to Health. Alliance Hospital, OASAS, and JC standards. Participate as a team member in conferences, treatment planning on behalf of patient. Attend in-service training and keeps professionally updated through reading and attending seminars. Provide lectures, workshop/training for patients and staff as assigned.
