Summary Will accept any Social worker with a Masters or Bachelors Degree----- **May be eligible for sign on bonus up to $5,000*** This role covers the Alma area. Responsible for providing psychosocial evaluations and treatment under the direction of a physician's plan of care to patients who are receiving home care services. Responsibilities (40%)* Provides social work interventions to patient and family including brief therapy, counseling, education, crisis management, pt-centered decision making, advance directives, care planning, resource linkage, funeral planning, etc. (25%)* Evaluates the patient's social, emotional, spiritual, financial, and environmental factors related to their illness, response to treatment, and adjustment to care. (25%)* Collaborates with physician and other team members regarding significant social and emotional factors that affect the medical plan of care. Advocates and negotiates with health care team and community agencies for patient/family to receive needed care. (10%)* Develops a social work treatment plan based on assessment and input from patient, family, and other disciplines involved in care. OTHER DUTIES AND RESPONSIBILITIES: Assesses special needs related to age, cultural diversity, developmental issues of patient/family including communication, space, role of family, special traditions, obstacles to learning, etc. Addresses safety and environmental issues including abuse, neglect, violence and/or suicide. Consults with agencies regarding patient needs and makes referrals as appropriate. Educates patient and/or family with information about community resources and assists with accessing resources as needed. Identifies the need for and conducts family meetings that result in understanding, comfort, decision making and other important outcomes. Assists patient/family in financial matters impacting treatment plan. Provides education regarding financial entitlement programs and links patient with appropriate agencies. Assesses patient and caregiver psychosocial status, family dynamics and communication patterns, identifying where additional counseling or specialized services or therapy is needed. Provides clear documentation of psychosocial problems, impact on treatment, MSW goals, and patient/family response to treatment. Maintains timely, thorough, concise documentation in all required systems. Completes initial evaluations, orders, visit notes, communication notes, case conference notes, IDT notes, recertifications and discharge summaries as appropriate. Integrates relevant theories of family dynamics, crisis intervention, strengths-based, solution-focused, and meaning of illness into everyday casework. Assists with transition to long-term care facility or inpatient setting as needed. Participates in case conferences, inservice programs and staff meetings as assigned. Participates in IDT and serves as counselor as needed. Facilitates support/education groups as needed. Provides consultations to agency staff members as needed. Observes strict confidentiality as well as respect for patient/family autonomy. Completes bereavement risk assessment and coordinates bereavement plan as needed. Assists in community education as needed. Builds and maintains positive relationships with patients, families, healthcare providers and community agencies. Provides excellent customer service. Willingly and consistently offers to help peers as needed. Communicates teamwork, integrity, caring and compassion. Assumes responsibility for professional development and education requirements for maintenance of professional licensure. Other duties as assigned. Education Specialization: SOCWRK Essential: true Other Information EXPERIENCE, TRAINING AND SKILLS: 1 year of social work experience in a health care setting. Access to reliable transportation. Ability to communicate effectively with all people and to express ideas and concepts in a clear and understandable manner. Ability to speak, write, and understand the English language. Ability to recognize needs and concerns of people and maintain constructive interpersonal relationships. Ability to work independently, demonstrating problem solving and organizational skills. Ability to relate to and work with ill, disabled, elderly, and terminally ill people in the home setting. Basic knowledge of community health, welfare, and social agencies. Adherence to the National Association of Social Workers Code of Ethics. Ability to drive under hazardous road conditions. Ability to work where employee may be exposed to communicable diseases in unpredictable home situations. Must be able to use all personal protective equipment correctly. Experience in Home Care preferred. PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS: Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Sedentary. Must be able to occasionally (0-33% of the workday) lift or carry 0-10 lbs.
