Description
The Clinical Social Worker in the Collaborative Care is responsible for providing brief interventions using evidenced-based techniques such as Behavioral Activation, Problem Solving Treatment, Motivational Interviewing, or other appropriate treatments in primary care settings. The role involves working collaboratively with PCPs and psychiatric consultants, supporting psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects, complications, and effectiveness of treatment. The Clinical Social Worker participates in regularly scheduled caseload consultations with the Psychiatric Consultant and communicates resulting treatment recommendations to the patient's PCP, particularly for patients with new treatments or those who are not improving as expected.
Responsibilities include documentation in the EMR and tracking patient follow-up/clinical outcomes in the CMTS registry. The Clinical Social Worker will facilitate referrals for clinically indicated services outside of the primary care clinic, enhance patient engagement, and ensure follow-up care. Additionally, they will complete relapse prevention plans with patients who are in remission.
QualificationsEducation: Master's Degree in Social Work, Mental Health Services.
License/Certification/Registration(s) Required: Licensed Clinical Social Worker, Licensed Mental Health Counselor, Certified Counselor, Licensed Marriage/Family Therapist, Licensed Psychologist, or Licensed NP.
Experience/Skills Requirement: 3+ years of relevant experience working with patients from underserved populations who have co-occurring behavioral health and physical problems. Bilingual (Spanish/English) required.
Computer literate with data entry capabilities. Knowledge of healthcare and chronic diseases, as well as Social Service programs and service plans.
