Job Summary

As a Social Worker in Case Management, you will:

  • Offer counseling and supportive services related to healthcare and home care programs.
  • Work collaboratively within an interdisciplinary team to address the social, emotional, and economic needs of patients and families, helping them achieve or maintain an optimal level of functioning.
  • Provide crisis intervention and assist families in navigating the complexities of medical situations.
  • Coordinate care among patients, families, and healthcare teams.
  • Demonstrate a deep understanding of human behavior and development stages.
  • Respond appropriately to suspected abuse, neglect, or violence according to ethical guidelines and state laws.
  • Develop effective care plans for patients and families using available resources from various agencies.
  • Maintain professionalism by actively participating in care conferences and serving as an advocate.
  • Exhibit strong critical thinking, problem-solving, and decision-making skills.
  • Communicate effectively with patients, families, and healthcare professionals, building strong rapport.
  • Engage diligently with healthcare teams to foster patient well-being.

Depending on the department, you may also provide social services for donors and transplant recipients.

Qualifications

- Bachelor's degree in Social Work from an accredited program is required. - A license in good standing in practice state(s) is necessary: - In Minnesota: Licensed Social Worker (LSW)
