Position Summary The Social Worker (SW) serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with a special focus on discharge planning, psycho-social needs assessment, care/resource management, and provider/insurance issues. Collaborates with the physicians, patients, their family members, and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family, and provider satisfaction, optimize systems/resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services. The Social Worker's decisions and actions with patients and families are determined in an ethical manner practicing compassion and respect for the inherent dignity, worth, and unique attributes of each person. Effectively communicates with patients/families, visitors, colleagues, physicians, leadership, and volunteers. All areas of practice and care delivery are provided to achieve and maintain top-decile results and outcomes for patient experience, quality, and patient safety. The Social Worker follows and adheres to UNC Health Appalachian's policies and procedures, regulatory requirements, and standards of behavior. The Social Worker practices within the practice and scope of their license. The Social Worker demonstrates the UNC Health Appalachian's Mission, Vision, and Values in action and approaches work in a way that reflects action, efficiency, and personal accountability. The Social Worker demonstrates professional conduct, effective communication, and effective completion of tasks. Effectively and professionally manages working relationships with coworkers, physicians, and all UNC Health Appalachian team members in a way that promotes a healthy work environment. Responds quickly and accurately to emergencies in the work environment. Performs duties under general supervision. Always assists coworkers, patients, families, and visitors regardless of the patient assignment, so long as the assistance is in-line with their scope of practice and license. Compensation is commensurate upon education, experience, certification, and internal equity. Hiring compensation range starts from $26.54 /hour. Position QualificationsMinimum Education
  • Bachelor's degree in Social Work from an accredited college or university.
Experience
  • Previous experience in Care Management is preferred.
Required Registration/License/Certification
  • Certification in Care Management is preferred but not required.
Knowledge, Skills and Abilities
  • Knowledge of and works within the scope of practice of a social worker.
  • Ability to communicate courteously and effectively with the public, patients, family, staff, and physicians.
  • Acts within the chain of command, utilizing positive verbal and non-verbal skills.
  • Competent in providing safe and effective patient therapy within limits of certification and hospital policies and procedures.
  • Knowledge of the continuum of care provided.
  • Ability to demonstrate exceptional customer service to patients, family, staff, and physicians.
Physical Demands
  • Lifts, positions, pushes, pulls, and/or transfers up to and greater than 50 pounds.
  • Manual dexterity and mobility.
  • Prolonged, extensive, or considerable sitting/standing/walking.
  • Bending, squatting, twisting, and turning.
  • Near acuity: ability to see clearly at 20 inches or less.
Working Conditions
  • Regularly exposed to the risk of blood-borne diseases.
  • Exposed to hazards from electrical/mechanical/power equipment.
  • Exposed to odorous chemicals and specimens.
  • Exposed to unpleasant elements (accident, injury, slippery surfaces, uneven surfaces), etc.
  • Handles emergency or crisis situations.
  • Subject to irregular hours (days, evenings, nights, weekends, holidays).
