Health Professional
Position Summary: Provides assessment and intervention to assist residents/families, and staff to assure that the emotional and psychosocial needs of residents are met/maintained on an individual basis and that resident's rights are upheld. The position is responsible for a social work caseload in addition to administrative duties assigned.
Essential Functions and Responsibilities:
- Seeks case supervision and consultation as appropriate.
- Participates in discharge planning, development and implementation of care plans, MDS and resident assessments.
- Complete MDS and resident assessments (RAPs) as required by Medicare, federal, and state regulation.
- Obtains assistance for resident by referring him/her to community resources.
- Develop preliminary and comprehensive assessments of the social service needs of each resident that identifies the problems/needs of the resident and goals to be accomplished for each problem/need identified.
- Participate in discharge planning, development, and implementation of social care plans, MDS, and resident assessments.
- Maintain a productive working relationship with other departments and supervisors to assure that an interdisciplinary focus of resident care is maintained.
- Complies with federal, state, and local regulations, Marwood's established policies and procedures, NASW Standards, and ethical guidelines.
- Performs other related duties as required and directed.
Education and Experience:
Bachelor's degree in social work from accredited educational institution.
State of Michigan licensure as Licensed Bachelor's Social Worker (LBSW).
Preferred: One year health care experience.
