Social Services Worker - Eben Ezer Lutheran Care Center : Job Details

Assuring residents psychosocial needs are met through the care plan process. Essential Job Functions: Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center. Shall provide orientation to new residents and their families (including explanation of residents rights, advanced directives, house rules, etc.). Participate in the admission process, including assessments of potential residents as required. Addressing psychosocial needs of individuals or groups, either directly or by referral to community agencies as needed. Assist residents and families in coping with the medical and psychosocial aspects of residents illness and disability as well as the stay in the care center. Communicate with residents families when concerns or needs arise. Provide appropriate referrals to community services such as mental health. Coordinate transfers (room changes) within the care center and assist resident in adjusting to the transfer. Facilitate resident groups as needed.
