Social Services Director - Generations at Regency, LLC : Job Details

Social Services Director:Assist with planning, developing and directing our facilitys social service programs in accordance with current federal, state and local standards, guidelines and regulations. Complete all care plans. Complete the MDS (Minimum Data Set) within the Assessment Reference date. Ensure that medical, emotional, and social needs of each resident is met/maintained. Greet and orient new residents. Provide information regarding community resources to residents' families. Direct hospice and palliative care. Coordinate discharge planning. Facilitate regular care plan meetings with residents and their families. Address residents' grievances and assist nursing or other pertinent departments in resolving issues Requirements of the Social Services Director:Either a Bachelor's Degree in Psychology or Sociology, or a Bachelor's or Masters Degree in Social Work (MSW), or a LCSW Certificate.
