The NHC Member will also create healthcare and wellness educational materials, provide health counseling and coaching, and organize documentation and educational materials. The NHC Member will fill a much-needed gap in providing educational materials and resources related to public health services.About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client's towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.Shelter Health is one of 8 social medicine programs under the Whole Person Integrate Care umbrella. This position would be a vital part of care coordination, stabilization, and wrap-around services to adults experiencing homelessness.Major Duties and ResponsibilitiesEngaging in outreach and promoting health education topics and healthcare servicesCollaborating with pre-existing patients and partner organizationsCreating healthcare and wellness educational materialsProviding health counseling and coachingKeeping and organizing documentation and recordsCharacteristics of an Ideal CandidateTeam-orientedOrganizedComfort speaking with others, individually or in groupsAble to communicate effectively with all individualsCompassionateKnowledge Required for the PositionKnowledge of AmeriCorps/Health Corps member requirementsSkill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.SupervisionMember uses initiative in carrying out recurring assignments following set procedures, independentlyThe supervisor assigns service activities, advises on changes in procedures, and is available for assistance when requiredThe supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.ReviewThe service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.GuidelinesWritten and oral guides provide specific instructions for doing serviceGuidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situationsComplexityMember has to develop, analyze, or evaluate information before the service position can progressThe service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting dataIn making decisions, the member is often required to depart from past approaches and to extend traditional techniquesSpecial ConsiderationsPersonal vehicles are not required for this position.
