Job Description :
We are seeking qualified applicants for a Services Coordinator position, who will work with adults/children with Intellectual and/or Developmental Disabilities (I/DD) who are eligible for case management services.Responsibilities:
  • Provide case management services for adults/children with I/DD and their families/support team
  • Assist individuals, families/support team in accessing services and resources available
  • Developing, coordinating, and monitoring of Individual Support Plans (ISPs)
  • Work with partnering agencies, community organizations, school/educations programs, etc. to provide comprehensive service coordination to supported individuals
  • Serve as an advocate/liaison to assist individuals in obtaining and accessing necessary services including obtaining referrals or finding resources in the community
Qualifications:
  • Any combination of the following education/and or experience requirements
    • Bachelor's degree in behavioral science, social science, or a closely related field; or
    • Bachelor's degree in any field and one (1) year of human services related experience; or
    • Associate degree in behavioral science, social science, or a closely related field and two (2) years of human services related experience; or
    • High school diploma/equivalent and three (3) years of human services related experience
  • Good organizational and time management skills
  • Excellent verbal and written skills/communication
  • Valid driver's license
  • Knowledge of the Oregon Administrative Rules (OARs) is preferred
