Under general supervision, plans, manages, and evaluates social services programs. Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs.

Facilitates and participates in the development, review, and evaluation of Individualized Family Service Plans for infants and toddlers with developmental delays/medical conditions; assists families in gaining access to early intervention services; coordinates multiple providers of services and collaborates with community agencies and providers; coordinates developmental evaluations and assessments.

This position will be housed at the Baldwin County Health Department in Milledgeville, GA. Cases may be assigned from the 13 counties within the North Central Health District.

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; Two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or equivalent position. Note: Some positions require eligibility for licensure. Below are the Preferred qualifications for a Service Coordinator: 1. The potential candidate may have a bachelor or masters degree in one of the following fields.(Resumemustinclude date degree was awarded): Audiology Education Physical Therapy Child Development Elementary Education Psychology Child Psychology Family/Community Counseling School Psychology Clinical Psychology Human Services Social Work Developmental Psychology Nutrition Sociology Dietetics Nursing Speech-Language Pathology Early Childhood Education Occupational Therapy Special Education - OR - 2. The potential candidate may have a related bachelor's or masters degree(Resume must include the date degree was awarded). Related is defined asat least 20 percent of the credit hours of the transcript must address child development, disabilities, or family systems.A copy of the candidates full transcript(s)mustbe submitted with the resume. - OR - 3. The potential candidate may be a licensed Registered Nurse.A copy of the candidates Georgia nursing licensemustbe forwarded with the resume. ****In order to be considered, applicants must attach a copy of an completed college transcripts.AdditionalQualifications
  • Strong writing and organizational skills.
  • Ability to work collaboratively with families, other agencies, and early intervention professionals.
  • Knowledge of computer programs and ability to learn the agencys data management system.
  • Current GA Drivers License--Transportation to the office and appointments in the community.
