Service Coordinator

The Service Coordinator assists individuals with developmental disabilities such as mild/moderate intellectual disabilities, cerebral palsy, head injuries, autism, epilepsy, and certain learning disabilities in progress toward specific goals of the person-centered plan. Service Coordinator assists in improving the lives of persons with developmental disabilities through program services to enable those persons to live independently and productively. Service Coordinator provides services to individuals impacted by a variety of behavioral health issues, recognizing that the individuals we serve often times have experienced trauma that affects their development and adjustment. Service Coordinator demonstrates a warm and welcoming, empathic, hopeful attitude while conveying a philosophy of dual recovery. Service Coordinator will be trained in trauma-informed care and will work to help provide services in an environment that is sensitive and responsive which will prevent victimization, abuse, or trauma as a result of the care received. Service Coordinators provide targeted case management which is defined as a group of activities that assist individuals with developmental disabilities to gain access to the care and services they need, which may include assessment, planning, coordinating, monitoring, and documenting. Service Coordinator reports to Service Coordinator Supervisor.

Education
  • Bachelor's Degree with major specialization (24 semester hours) in social or behavioral sciences, special education, nursing, adjunctive therapies, counseling, vocational rehabilitation or closely related field
  • Master's Degree in social or behavioral sciences, special education, nursing, adjunctive therapies, counseling, vocational rehabilitation or closely related area.
Responsibilities
  • Planning of supports, including identifying the needs of the person being supported and creating an action plan that will support the person in meeting those needs.
  • Providing services designed to match the unique support needs of individuals with developmental disabilities and their families with resources in the community.
  • Ongoing monitoring and assessment of the quality, timeliness, and effectiveness of services and supports a client receives.
  • Documentation of time spent maintaining appropriate records in accordance with federal/state programs, policies, and procedures, including obtaining necessary releases and otherwise ensuring the confidentiality of all written and verbal discourse.
  • Providing services related to planning and coordination of the transfer or transition of care from one regional office to another or from the community to an institution.
  • Mentoring of another service coordinator by assisting with the facilitation and development of a person-centered plan.
  • Services related to terminating a person from the service delivery system.
  • Services related to securing a variety of services for individuals throughout their lives.
  • Perform all other duties as assigned.
Physical Requirements
  • Normal Office environment.
  • Typical physical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting up to 25 pounds.
  • Requires normal range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed.
  • Requires ability to work under stressful conditions and to work irregular hours and deal with irate individuals.
  • TB test upon hire.
