Join to apply for the Service Coordinator role at Concern Housing .Concern Housing is seeking a Service Coordinator responsible for assisting supportive tenants with community integration using a person-centered, strength-based approach. The role involves facilitating linkages with community resources, providing individualized counseling and support, and ensuring proper documentation and compliance with program requirements. The Service Coordinator is a vital resource and advocate for tenants with special needs.Responsibilities include but are not limited to:Facilitating tenants' linkage with community support services such as care coordination, behavioral and physical health services, vocational supports, entitlements, and peer supports.Using a person-centered approach to develop quarterly Support Plans, conduct Functional Assessments, and review Support Plans. Meeting with residents to develop skills and monitor progress. Responsible for documentation, including progress notes and discharge plans.Providing individual counseling to promote skill development, including wellness management, medication independence, housing readiness, and financial skills.Employing harm reduction strategies to improve health outcomes and monitor progress toward healthcare goals.Supporting the availability of housing services, including addressing maintenance issues.Attending weekly supervision and staff meetings.Performing other duties as assigned by the Program Director or Supervisor.Remaining current with all required compliance documents and training.Salary: $55,000Position Requirements:Bachelor's Degree in Psychology, Rehabilitation, Social Work, or related field.At least 2 years of experience in a similar setting or working with individuals with mental illnesses.Strong verbal and written communication skills and sound judgment regarding client care.Concern Housing offers a comprehensive benefits package, including medical insurance (80% premium paid by the agency), dental and vision insurance, flexible spending accounts, retirement plans, life insurance, generous paid time off, educational assistance, licensure reimbursement, internship/clinical supervision opportunities, and referral bonuses.About Concern Housing: A non-profit organization dedicated to supporting individuals and families to live with dignity through housing and support services, serving approximately 1,300 individuals across over 240 locations in New York State.
