Service Coordinator, Family Networks - Beth Israel Lahey Health : Job Details

Are: Receive case assignments. Set up Family Team Meeting (FTM) as required by service type. Create service referrals. Enter meeting notes into Family Networks dictation. Assist with duty day one-two days per week. Secure emergency placements when necessary. Communicate with Data Coordinator for changes in services via data form/email. Communicate ongoing cases with Providers and Social Workers and discussing role of service model-short term/least restrictive/ etc. Assist area staff with identifying community resources, brainstorming/problem solving supports for families. Attend meetings as needed. Qualifications:Bachelors Degree in Psychology or related field required and two years of direct service experience in child and family services preferred.
