About the Service Coordinator - Case Manager Role As a Service Coordinator within our Early Intervention team, you'll play a vital role in supporting children under the age of 3 who have developmental delays by coordinating access to needed services and supports. This position is ideal for someone who is compassionate, organized, and culturally responsive, and who thrives in fast-paced environments where collaboration matters. Starting Rate: $20.00/hour, based on experience and qualifications Service Coordinator - Case Manager Core Responsibilities
- Request and complete service authorizations in a timely manner
- Conduct Family Service Plan meetings and ensure compliance with OCDEL standards
- Coordinate services and make referrals to support each child's development
- Participate in Multi-Disciplinary Team meetings to promote collaborative planning
- Develop and update transition plans to maintain continuity of care
- Maintain positive and professional relationships with families, providers, and community partners
- Document all services and communication accurately and on time
What You Bring
- Bachelor's degree in Early Childhood Education, Psychology, or a related field and at least 1 year of experience in a human services role;OR an associate's degree or 60 college credits in Early Childhood Education and 3 years of relevant experience
- Strong customer service and time management skills
- Ability to navigate digital systems and prioritize multiple tasks
- A positive, solution-focused attitude and ability to build rapport
- A valid driver's license and reliable transportation
