UAS Assessor HealthCare Support is actively interviewing for a Full-Time and Part-Time Senior UAS Assessors to join our care management team in Bronx, NY. You will make a meaningful impact on your community. In this field-based role, you'll conduct initial assessments for members and play a vital part in shaping their care journey. You'll work independently in the field, supported by an administrative team that schedules your visits and coordinates logistics, so you can focus on providing high-quality care and advocacy. - Full-Time: Monday-Friday, 8:30am-5:00pm (37.5 hrs/week)
- Part-Time: 3 days/week - Mon-Wed or Wed-Fri (22.5 hrs/week)
- No weekends, holidays, or night shifts

Daily Responsibilities for UAS Assessor:
- Conduct initial and reassessment UAS-NY evaluations for new and existing members
- Develop and update personalized care/service plans
- Identify and coordinate necessary services, resources, and specialists
- Monitor member progress and update plans as needed
- Collaborate with care teams, intake coordinators, and community resources
- Educate members and families on available services and benefits
- Document all care management activities per state, federal, and organizational guidelines
- Work independently in the field, making critical clinical decisions to support members' needs
Required Qualifications for UAS Assessor :
- Active, clear NY State RN license
- 2+ years UAS, home health, or homecare experience (OASIS, CHHA, or MLTC may be considered)
- Reliable transportation (public transit, personal vehicle, or ride services)
