Job Type Full-time Description Overview: The Senior Network Executive - Post-Acute Partnerships is a senior leadership role responsible for expanding and optimizing the post-acute provider network that supports our value-based care and care management programs. This position leads the identification, engagement, and contracting of skilled nursing facilities (SNFs), home health agencies (HHAs), inpatient rehab facilities (IRFs), and long-term acute care hospitals (LTACHs), ensuring alignment with clinical goals, quality standards, and shared savings initiatives. The VP works closely with internal teams-including clinical operations, analytics, legal, finance, and client success-to structure sustainable partnerships that support performance improvement, network integrity, and member outcomes. This individual is both a strategist and an operator, capable of leading negotiations, presenting business cases, and building trusted relationships with executive and clinical leadership at external provider organizations. Primary Responsibilities and Essential Duties:

  • Develop and execute a scalable strategy to build a high-performing post-acute provider network in alignment with company VBC goals.
  • Identify high-impact market opportunities using data on referral patterns, leakage, quality performance, and cost variation.
  • Lead outreach in targeted geographies, balancing growth goals with quality and access standards.
  • Serve as the primary point of contact for post-acute provider engagement, educating potential partners on value-based models, expectations, and benefits.
  • Negotiate contracts that incorporate performance incentives, data-sharing expectations, and aligned workflows.
  • Build long-term, collaborative relationships with PAC administrators, executives, and clinicians.
  • Work cross-functionally with legal, finance, care management, and technology teams to ensure smooth contract execution and integration.
  • Partner with provider implementation and client success teams to support a seamless transition from contracting to go-live.
  • Provide feedback from the field to help refine service offerings, workflows, and performance standards.
  • Maintain an up-to-date pipeline and track engagement metrics, deal progression, and network gaps.
  • Contribute to reporting on post-acute network adequacy, performance trends, and savings opportunities.
  • Support executive reporting and participate in strategic planning efforts across the growth and clinical teams.
  • Build and lead a high-performing business development team focused on expanding and managing the post-acute provider network.
  • Define team structure, roles, and hiring priorities to support national and regional growth objectives.
  • Recruit, onboard, and train team members in value-based care principles, post-acute market dynamics, and effective provider engagement strategies.
  • Establish clear performance expectations and KPIs for team members, including outreach volume, contract progression, and network adequacy targets.
  • Provide ongoing coaching, feedback, and professional development to ensure continuous growth and alignment with company goals.
  • Foster a collaborative and mission-driven team culture that prioritizes accountability, communication, and results.
Requirements Qualifications
  • Bachelor's degree required; Master's degree in Business, Healthcare Administration, or Public Health preferred.
  • 5+ years of experience in healthcare business development, network contracting, or provider relations.
  • Strong working knowledge of post-acute care delivery, operations, and reimbursement.
  • Experience negotiating contracts with SNFs, HHAs, IRFs, or LTACHs is strongly preferred.
  • Familiarity with value-based care models (MSSP, MA, BPCI-A, TEAM, ACOs) required.
  • Excellent communication, presentation, and interpersonal skills across clinical and executive audiences.
  • Proven success in provider engagement and closing healthcare partnership deals.
  • Ability to manage complex projects, timelines, and stakeholders with minimal oversight.
  • Strategic thinker with an operational mindset and attention to detail.
  • Proficient in using CRM platforms, data tools, and Microsoft Office Suite.
