Compass Healthcare Consulting & placement is conducting a search for an experienced Senior Intake Coordinator for a Licensed Home Care Services Agency in Brooklyn, NY. This is a great opportunity for an experienced Home Care Intake Coordinator to get in with a great company! Qualified candidates will have prior Intake Coordination experience within a Licensed Home Care Agency, will understand the verification and Intake processes and procedures of a Home Care Agency. Qualified candidates will have prior experience completing Intake referrals and using HHA Exchange Program.

Senior Intake Coordinator - Home Care - LHCSA

Qualifications:

• Experienced with HHA Exchange Program
• Experienced with Home Care LHCSA, Insurance Verification process
• Experienced with Licensed Home Care Services Agency Policy & Procedure
• Prior Intake Experience
• Must have prior Home Care Intake Coordinating Experience (Not an RN)
• Experience working in a LHCSA, CHHA, MLTC or Home Care Related Field

Job Responsibilities:

• Coordinating Home Care Referral Process from intake to service delivery
• Collaborating with Business Development, Coordination, RNs and Field Staff
• Completing referral information required for Intake
• Other related tasks.
• Home Care Intake Coordination Experience and Skills required.
