Your Role as a Case Manager:
- Collaborate with an interdisciplinary team to develop and guide treatment plans based on patient needs.
- Coordinate with the team to create provisional discharge plans and ensure contingency plans are in place.
- Manage the patient experience by monitoring quality, timeliness, and fulfillment of expectations.
- Facilitate weekly team conferences and make necessary modifications to treatment plans.
- Complete case management documentation accurately and in accordance with regulations.
- Analyze case management reports and determine appropriate actions based on Key Care Indicators.
- Understand and navigate commercial contract levels, payor requirements, and recertification needs.
- Attend Acute Care Transfer (ACT) meetings to identify trends and collaborate on improvement strategies.
- Meet with patients and families within 24 hours of admission to discuss goals and care expectations.
- Assist patients with timely access to resources to prevent discharge delays.
Qualifications You Need:
- License or Certification: Must be qualified to independently conduct assessments in your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling).
- If your discipline requires licensure, you must hold an active license.
- Eligibility for CCM or ACM certification upon entering this position or within two years is required.
- For nursing roles, a minimum of an Associate Degree in Nursing with active RN licensure is essential; a BSN is preferred.
- For other eligible licensed or certified health care professionals, a minimum of a bachelor's degree is required; graduate degree preferred.
- At least 2 years of rehabilitation experience is preferred.
