Position Objective: To provide best practices, training, support and monitoring to Career Counselors. Responsible for completing a comprehensive intake, assessment and service plan for clients referred to PACE. Identify strategies and tools for assisting clients in determining their career pathway and education planning. Assist in the development of relationships with partner programs to strengthen the referral process for program clients to educational programs, vocational training, community service and internship opportunities. Essential Functions: •Work with PACE participants who need intensive vocational career counseling and other services due to their challenges. Conduct intake and internal assessment to determine client's assigned Service Tier (Tier 3). •Develop a comprehensive service plan based upon client intake, assessment results, and thorough understanding of the individual's interests, strengths, employment and training history, preferences, and needs. This plan will be completed within 30 calendar days of client enrollment to PACE. •Assist with the assignment of program participants into Career Education Services, Alternative Engagement Opportunities, Internship and/or Community Service, Bridge programming as appropriate, Job Placement or other HRA Programs/services. Meet monthly goals identified in this area. Additionally, utilize program flyers, information sessions, link system and informational emails for this purpose. •Complete HRA's on-line employment plan (EP) and individualize service for each enrolled client. •Taking the results of the TABE or SABE and other types of assessment instruments used in PACE, determine the educational, skills level, and vocational interest of the participant. •Conduct a one-on-one interview to collect information about the participant's personal work history, barriers to employment, criminal history, personal/family background, and living situations. Identify barriers and issues that may result in participant being returned to HRA for further evaluation. •Provide guidance to Career Service Coordinators as they work to develop a comprehensive service plan based upon a thorough understanding of the clients' interests, strengths, preferences and needs. •Collaborate with Community Partner Liaison to identify slot availability of trainings, internships, community service and educational opportunities for client referrals, Alternative Engagement and Concurrent Assignments and other HRA programs/services. •Assist the Supervisor of Service Coordination and Deputy Director with auditing client files and the electronic case management system to ensure documentation and case notes are entered according to program expectations. •Maintain and execute documentation and workflow through computer-based programs and technology that draws on basic software/computer skill set. •Collaborate with other PACE staff to coordinate and facilitate employment readiness, placement and follow-up services. •Performs other duties as assigned.
