Massapequa, NY 11758 Shift timing:- 8:00am - 3:15 pm Student Details: Name of Student: Olivia Stupnikov Date of Birth: 11/27/2020 Grade: kindergarten Diagnosis: Type 1 Diabetes Mellitus Duties: Assist the student in monitoring their medication and overall well-being, including behavior associated with diabetes.Description:

• Regularly monitor the student's blood glucose levels and respond appropriately to fluctuations.
• Administer insulin or other prescribed medications as needed, following established care plans.
• Ensure that the student's medication is administered accurately and on schedule.
• Communicate with parents/guardians and healthcare providers about the student's medication needs and any changes in condition.
• Observe and assist the student with behaviors that may be related to blood sugar fluctuations, ensuring their comfort and safety.
• Provide support and reassurance to the student during episodes of hypoglycemia or hyperglycemia.
• Work closely with teachers and school staff to ensure the student's needs are met in the classroom environment.
• Keep emergency contact information and supplies readily available at all times.
• Collaborate with school staff, parents/guardians, and healthcare providers to ensure comprehensive care for the student.
• Participate in meetings and provide updates on the student's health status and care plan.
• Educate school staff on the student's condition and how they can support the student's needs.

Certification:

• Active LPN license
