Job Summary: As a Program of All-inclusive Care for the Elderly (PACE) Scheduler, you will play a crucial role in ensuring the smooth and efficient coordination of participant appointments. Your attention to detail, excellent communication skills, and ability to manage multiple tasks will contribute to the overall success of our program in providing comprehensive care to the elderly population.

Duties/Responsibilities

Essential Job Functions:

  • Appointment Scheduling: Coordinate and schedule medical, dental, therapy, and other necessary appointments for program participants while considering individual preferences and healthcare requirements.
  • Transportation Coordination: Collaborate with transportation manager to optimize scheduling and address any logistical challenges.
  • Participant Communication: Maintain regular communication with program participants, their families, and caregivers to inform them about upcoming appointments, changes in schedules, and any special requirements.
  • Authorizations: Liaise with outside providers, communicating regarding authorization information for appointments.
  • Multi-Disciplinary Team Collaboration: Collaborate closely with medical professionals, care coordinators, and other team members to align appointment schedules with participants' overall care plans.
  • Electronic Record Keeping: Utilize electronic health records (EHR) systems to accurately document and maintain up-to-date records of scheduled appointments, transportation arrangements, and related communications.
  • Resource Management: Monitor and manage the availability of medical equipment, facilities, and personnel needed for appointments. Coordinate with relevant departments to ensure all necessary resources are prepared.
  • Problem Solving: Proactively identify and address scheduling conflicts, transportation challenges, and participant preferences to ensure the highest level of participant satisfaction and adherence to care plans.
  • Data Analysis: Collect and analyze scheduling data to identify trends, optimize scheduling processes, and contribute to program improvement initiatives.
  • Regulatory Compliance: Adhere to regulatory guidelines and privacy regulations when handling participant information and scheduling healthcare services.
  • Continuous Improvement: Participate in team meetings, trainings, and performance improvement projects to enhance scheduling efficiency, participant experience, and overall program effectiveness.

    Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Sitting, Standing, Walking, Talking, sitting at a desk, and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands: Sedentary physical activity performing non-strenuous daily activities of an administrative nature.

  • Minimum Educational and General Qualifications: High school diploma or equivalent; Associate degree or higher in healthcare administration or related field preferred. Prior experience in scheduling, preferably in a healthcare or social services setting. Strong organizational skills and attention to detail to manage complex schedules and coordinate various appointments. Excellent communication skills, both written and verbal, to effectively interact with participants, caregivers, and interdisciplinary team members. Proficiency in using electronic health record systems and scheduling software. Empathy and compassion when dealing with elderly participants and their families. Ability to work independently and as part of a team in a fast-paced, dynamic environment. Knowledge of medical terminology and regulatory requirements related to healthcare scheduling and participant privacy.
