This is a hands-on leadership role where you'll guide a dedicated team, ensure smooth admissions and discharges, manage complex patient needs, and keep your unit survey-ready at all times.You'll have the autonomy to lead your team, the backing of a collaborative leadership group, and the satisfaction of knowing your work directly impacts patient outcomes and family satisfaction.Key Responsibilities
- Supervise and support a team of nurses and CNAs (10-12 nursing staff across 1st and 2nd shifts)
- Oversee all admissions, discharges, and complex patient care needs on the skilled rehab unit.
- Manage the MDS process for skilled residents, including MTAs for revenue optimization.
- Coordinate and attend weekly discharge planning meetings with residents and families.
- Coordinate with the Scheduler on staffing and address absences while minimizing overtime.
- Serve as administrative authority during shifts, escalating urgent concerns as needed.
- Provide compassionate, solutions-focused communication with residents' families to ensure trust and satisfaction, including service recovery when needed.
- Supervise patient diets, fluid intake, and respond to changes in condition.
- Lead incident investigations and report findings to ADON.
- Approve timecards and maintain accurate staff records.
What We're Looking For
- Current Georgia Nursing License
- RN preferred, LPNs considered for strong candidates.
- 1+ years in skilled nursing or case management with at least one successful survey.
- Experience in long-term care (hospital-only backgrounds are not equivalent).
- MDS knowledge and survey readiness experience required.
- Supervisory experience preferred (may waive for strong internal candidates).
- CPR certification.
- Strong understanding of rehab administration, discharges, and MDS process.
