Job Title: RN Case Manager - 13-Week Contract Location: White Plains, NY Start Date: June 9, 2025 Shift: Day Shift (Full-Time) Duration: 13 Weeks Job Type: Contract Key Responsibilities:

• Collaborate with physicians, healthcare providers, and the interdisciplinary team to ensure timely and appropriate care plans for patients.
• Conduct assessments and develop individualized discharge plans to facilitate safe transitions of care.
• Monitor and coordinate the patient's progress through the continuum of care to ensure cost-effective and quality outcomes.
• Evaluate patient needs, available resources, and insurance coverage to make informed case management decisions.
• Act as a liaison between patients, families, and healthcare providers to ensure clear communication and alignment on care goals.
• Maintain accurate documentation in compliance with regulatory and facility standards.
• Ensure timely and efficient utilization of services while advocating for the patient's best interests.

Requirements:

• Active RN license in the state of New York
• Minimum of 2 years of recent RN Case Management experience in an acute care setting
• Strong knowledge of discharge planning, utilization review, and insurance coordination
• Excellent communication, critical thinking, and organizational skills
• Experience with electronic medical records (EMR) systems preferred
• Case Management certification (ACM or CCM) is a plus
