Develop, implement, and adjust individualized care plans in partnership with patients and their families. - Health Advocacy: Promote and protect the health, safety, and rights of patients, ensuring a respectful and trusting environment is maintained among all team members. - Patient Education: Provide comprehensive education to patients and families, empowering them to understand self-care practices post-hospitalization. Tailor educational sessions to accommodate different learning styles and assess retention of information. - Quality Improvement: Participate in systematic evaluations of nursing practices and implement changes for quality enhancement. Engage in self-reflection and professional development to strengthen nursing skills. - Communication & Support: Foster meaningful connections with patients and families, ensuring that their needs and concerns are at the forefront of care delivery. Create a healing environment that promotes comfort and emotional well-being. - Medication Administration: Demonstrate proficiency in administering medications and implementing specific nursing interventions as needed. Qualifications: - Current New York State RN Licensure. - BLS and ACLS certifications (AHA or RQI only). - One (1) year of experience in a Med/Surg environment as a registered nurse. - Excellent interpersonal and communication skills with a strong ability to collaborate as part of an interdisciplinary team. Work Schedule: - Full-time position requiring 36 hours per week plus 2 on-call shifts within a 4-week period. - Candidates must be available to work holidays and throughout the holiday season (18-26 weeks with longer preferred).
