Key Responsibilities: - Conduct comprehensive assessments of patients and families to identify needs and concerns. - Develop and implement individualized, culturally relevant care plans in collaboration with patients, families, and the healthcare team. - Monitor patient progress and adjust care plans based on patient responses and changing conditions. - Provide direct patient care, including medication administration and monitoring of vital signs. - Educate patients and their families on self-care practices and necessary lifestyle changes for health and recovery. - Foster a healing environment by building trust and rapport with patients and their families. - Participate in quality improvement initiatives and engage in self-evaluation to enhance professional development. - Collaborate with healthcare team members to deliver comprehensive patient care and promote a supportive team atmosphere. - Ensure compliance with infection prevention strategies and maintain required competencies. - Conduct regular patient rounds and provide necessary information and tools to enhance patient comfort and safety. Qualifications: - Current New York State RN Licensure. - Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications required. - Minimum of one (1) year of experience as a Registered Nurse in a Med/Surg environment preferred. - Strong critical thinking and problem-solving skills. - Excellent communication and interpersonal skills to effectively engage with patients, families, and team members.
