Collect data with regard to residents physical and mental health including reaction to illness and treatment regimens. Record and report results of nursing assessment to physician, other personnel and responsible party as appropriate. Deliver and record licensed nursing care to include medications and treatments as prescribed by the physician as well as care outlined in the residents individual plan of care (ADLs, nutrition, hydration, labs retrieval/oversight, weight oversight, therapies and restorative initiatives, splinting, assistive devices, skin audits, behavior logs, acute episodic oversight documentation, etc.). Coordinates and provides oversight to non-licensed personnel in implementing the residents individual plan of care. Ensure continuity of care through documentation and communication of resident status (report) to relieving personnel.
