What You'll Do• Complete and coordinate MDS assessments• Develop and update individualized care plans• Monitor resident progress and support QAPI efforts• Communicate with residents, families, and providers

What You'll Bring• Current RN license• Knowledge of MDS 3.0, RAI, and care planning• Strong communication and organizational skills• RAC-CT certification preferred
