RN Intake and Referral Specialist - St. Charles Health System : Job Details

The Intake and Referral Specialist maintains daily contact with physicians, case managers, social workers, discharge planners, patients, families, caregivers, home health and hospice agencies and other referral sources in the hospital and the community regarding potential referrals to home health services. Markets the services of the home health department. Develops relationships with physicians, providers, skilled nursing facilities, payers and others in Central Oregon. Prepares patients and families for admission to the home health department, and facilitates that process. This position does not directly manage any other caregivers. ESSENTIAL FUNCTIONS AND DUTIES:Accepts service referrals and clarifies needed services where possible (via fax, telephone, or direct contact).
