This full-time contract position provides flexibility, stability, and the chance to make a meaningful impact in patients' lives—all while enjoying the support of two respected healthcare organizations.ResponsibilitiesPosition Summary:The RN Hospice Case Manager delivers nursing care to patients in their homes and coordinates their treatment plans with a supportive, multidisciplinary team. This role blends hands-on care with case management responsibilities in a rewarding, patient-centered environment.Key Responsibilities

• Provide direct, skilled nursing care to patients in the home setting
• Conduct initial assessments and develop individualized care plans
• Evaluate and revise plans as needed, collaborating with team members
• Administer medications, perform wound care, IV therapy, and other clinical procedures
• Educate patients and caregivers on treatments, medications, and disease processes
• Ensure timely, accurate clinical documentation of care provided
• Verify patient eligibility and compliance with home health guidelines

Qualifications

• Active RN license in the state of Massachusetts
• Minimum 2 years of nursing experience
• At least 1 year of home health experience preferred
• Strong communication, assessment, and critical thinking skills
