RN Home Health - A-Line Staffing Solutions : Job Details

Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assess home health and/or hospice patients in stable and emergent situations. Implement, develop, document, and evaluate the Plan of Care (POC). Maintain up-to-date patient records and assure continuity of quality patient care. Provide care utilizing infection control measures. Educate patients and/or their families regarding care, diet, medications, and home safety. Supervise and provide clinical directions to HHA/ STNA/ LVN. Monitor assigned cases to ensure compliance with requirements of third party payer. Perform on-call responsibilities and provide on-call services as assigned. Attend interdisciplinary team meetings and perform other related duties as assigned. Critical Information Knowledge of medications and their correct administration.
