RN - Group Home Nurse Case Manager - Yonkers & Carmel - Northwell Health : Job Details

13 Week Contract, Full-Time Monday-Friday 8:30 A-5:00 P. Will be required to cover Carmel & Yonkers Locations. POSITION DUTIES AND RESPONSIBILITIES:include but are not limited to the following: 1. Coordinate the delivery of routine and specialty health care services to address the assessed needs of persons receiving services as assigned and maintain health care profiles. 2. Assess and establish health care outcomes for persons receiving services. Updates assessments on at least a semiannual basis or as indicated. 3. Coordinate with the Medical Case Manager Counselors (MCMC) to ensure medical, dental, and specialty appointments, consults, visit documentation, and follow-up appointments are tracked and scheduled according to requirements. 4. Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics, and all other service providers. 5.
