\n \n\n \n\nThe role you'll contribute:
\n \n\n \nResponsible for coordinating the efforts of the multidisciplinary treatment team to provide comprehensive care for patients. This position administers, supervises, coordinates, and evaluates the nursing process to maintain and improve patient outcomes. Additionally, this role ensures compliance with regulatory standards, established policies, protocols, and procedures. In addition, this position collaborates with other healthcare professionals to deliver comprehensive care and utilizes pertinent references as guidelines for clinical practice. Responsibilities for this role include supervising and teaching other personnel, administering medications and treatments, and critically evaluating evidence for application to clinical practice.

\n \n\n \nConduct comprehensive clinical assessments to evaluate patients' physical, mental, and emotional health.
\n \n\n \nDevelop and implement individualized care plans based on assessment findings and patient needs.
\n \n\n \nCoordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists.
\n \n\n \nMonitor patient progress, regularly review, and adjust care plans based on patient responses and outcomes.
\n \n\n \nCommunicate effectively with patients, families, and the healthcare team to ensure everyone is informed and involved in the care process.
\n \n\n \nDocument patient information accurately, including assessments, care plans, interventions, and outcomes in the patient's medical record.
\n \n\n \nAdminister medications and treatments safely, ensuring adherence to protocols and patient safety.
\n \n\n \nEducate patients and families about diagnoses, treatment options, and care plans to promote understanding and compliance.
\n \n\n \nSupervise and direct the activities of nursing and support staff, ensuring adherence to care standards and evaluating performance.
\n \n\n \nEvaluate discharge planning needs and coordinate patients' discharge requirements to ensure a smooth transition from hospital to home or another care facility.
\n \n\n \nEnsure compliance with regulatory standards and established policies, procedures, and protocols.
\n \n\n \nAddress and resolve issues related to patient care, staffing, and resource allocation, implementing solutions to improve processes and outcomes.
\n \n\n \nPerforms other duties as assigned
\n \n\n \nThe expertise and experiences you'll need to succeed :
\n \n\n \nMinimum qualifications :
\n \n\n \nAssociates Degree in Nursing Required
\n \n\n \nBachelors Degree Preferred
\n \n\n \nRN licensed in the state of TX or qualifying compact state
\n \n\n \n1 Year of Experience Required\n \n\n \nBLS, ACLS, PALS Required
\n \n\n \nTNCC Required
\n \n\n \nNIHSS Required
