Employment Type:Full timeShift:Description:Job Title: Care Coordinator RNLocation: THWMI - Muskegon HospitalDepartment: Rehab UnitSchedule: Full-time Day Shift Job Summary:The Care Coordinator RN is a licensed professional nurse who works collaboratively with physicians, the interdisciplinary care team, and patients/families to coordinate care across the continuum. This role is responsible for assessment, planning, monitoring, and evaluation of patient care throughout an episode of illness—ensuring quality outcomes, cost-effectiveness, and a smooth transition between care settings.Essential Functions:
- Coordinates patient care using clinical pathways, case management tools, and interdisciplinary conferences.
- Assesses patients using evidence-based criteria (e.g., InterQual) and identifies appropriate levels of care.
- Monitors resource utilization, outcomes, and patient progress.
- Facilitates communication among healthcare teams, patients, and families.
- Acts as a resource for care coordination, discharge planning, and education.
- Participates in quality improvement and care redesign initiatives.
- Maintains thorough documentation in the electronic health record system.
Minimum Qualifications:
- Graduate of an accredited nursing program
- Bachelor's degree in Nursing (BSN) required; Master's degree preferred or in progress
- Current RN license in the State of Michigan
- Minimum 5 years of clinical RN experience; recent case management experience required
- At least 1 year of experience within Trinity Health preferred
- Strong communication, critical thinking, and interpersonal skills
- Knowledge of utilization review, payer systems, and regulatory compliance
- Proficient in electronic medical records and MS Office tools
Physical & Environmental Requirements:
- Regular walking, sitting, standing, and reading
- Occasional lifting or moving up to 20 lbs
- Exposure to patient care environments and clinical risk factors
