Position Overview: Registered Nurse (RN) will be responsible for conducting comprehensive resident assessments to evaluate their health needs, monitor progress, and adjust care plans as necessary. This role requires a skilled nurse who can perform detailed assessments, identify potential health issues, and work closely with the healthcare team to provide high-quality care to our residents Key Responsibilities:
- Conduct detailed and accurate initial and ongoing resident health assessments, identifying medical, physical, and emotional needs.
- Monitor and document changes in residents' conditions, reporting findings to the medical team.
- Update and review care plans based on assessment results to ensure optimal care for each resident.
- Administer medications, treatments, and other nursing interventions as prescribed by physicians.
- Collaborate with physicians, care staff, and families to develop personalized care plans for each resident.
- Provide health education to residents and their families regarding conditions, treatments, and self-care.
- Ensure that nursing assessments and other documentation comply with regulatory standards and facility policies.
- Respond to emergency situations promptly and provide critical care when necessary.
- Participate in care team meetings to discuss care updates and changes in residents' health status.
Qualifications:
- Current and valid Registered Nurse (RN) license in NY.
- Previous experience in assisted living, long-term care, or geriatric nursing preferred.
- Experience in conducting resident health assessments and developing care plans.
- Current BLS/CPR certification.
- Strong clinical assessment skills and attention to detail.
- Excellent communication skills, with the ability to effectively collaborate with residents, families, and the healthcare team.
