The Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition for patients and through ensuring that patients are at a medically appropriate level of care.Position QualificationsEducation
- Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment
- Case Management Certification preferred
Experience:
- Five years Clinical RN experience required
- Diverse knowledge of community services, resources and programs
- Previous case management experience preferred
Special Skills and Competencies:
- Excellent interpersonal communication and negotiation skills
- Strong analytical, data management and PC skills
- Ability to establish and maintain working relationships
- Ability to work independently and exercise sound judgment in interactions with physicians, payers, associates and patients/families
- High degree of initiative and judgment
- Ability to work flexible hours
- Able to set and organize own work priorities and adapt to changes and interruptions
- Mandatory Child and Dependent Adult Reporting Education or completes requirement within 6 months of hire renews every 5 years
- Must provide proof of Basic Life Support- American Heart Association (BLS Healthcare Provider or HealthStream Heart Code) or American Red Cross via current card prior to employment
- Maintains current certification in BLS
