Employment TypePart timeShiftDay ShiftDescriptionPay Range: $26.48 - $40.11Job SummaryThe Case Manager collaborates with physicians, social services, nurses, and community agencies to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition for patients and through ensuring that patients are at a medically appropriate level of care.Position QualificationsEducation

• Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment
• Case Management Certification preferred

Experience:

• Five years Clinical RN experience required
• Diverse knowledge of community services, resources and programs
• Previous case management experience preferred

Special Skills and Competencies:

• Excellent interpersonal communication and negotiation skills
• Strong analytical, data management and PC skills
• Ability to establish and maintain working relationships
• Ability to work independently and exercise sound judgment in interactions with physicians, payers, associates and patients/families
• High degree of initiative and judgment
• Ability to work flexible hours
• Able to set and organize own work priorities and adapt to changes and interruptions
• Mandatory Child and Dependent Adult Reporting Education or completes requirement within 6 months of hire renews every 5 years
• Must provide proof of Basic Life Support- American Heart Association (BLS Healthcare Provider or HealthStream Heart Code) or American Red Cross via current card prior to employment
• Maintains current certification in BLS
