JK Executive Strategies is proud to partner with a nonprofit that specializes in services for adults with intellectual and developmental disabilities:

They are looking for a compassionate, skilled, and community-minded Registered Nurse to lead a unique initiative focused on enhancing health equity for adults with intellectual and developmental disabilities. This grant-funded position is designed to combine traditional RN duties with a leadership role in health education and community outreach.

The RN will work on-site at their residential campus, providing direct medical care and coordinating services for residents, while also serving as a bridge between the IDD community and local healthcare providers. The role is split evenly between clinical care and advocacy, training, and education.

Responsibilities

  • Clinical Nursing Duties (50%)Provide high-quality, person-centered nursing care to residents with I/DD, including medication management, routine assessments, and responding to medical concerns.
  • Support and coach Licensed Practical Nurses (LPNs) and Direct Support Professionals (DSPs) on best practices related to healthcare coordination and resident support.
  • Assist residents with preparation for medical appointments and follow-up from hospital discharges.
  • Contribute to internal quality improvement initiatives including the Safe Patient Handling Committee and Clinical Review Team.
  • Maintain accurate health records, document medical interventions, and support the development of individualized care plans.

  • Health Education & Community Outreach (50%)Design and deliver 1:1 and small-group health literacy workshops for residents focusing on appointment preparation, communication skills, and informed consent.
  • Develop accessible, plain-language educational materials for both residents and healthcare providers.
  • Conduct outreach to local healthcare organizations (hospitals, behavioral health centers, primary care offices) to build awareness of I/DD-specific care needs.
  • Educate external providers on inclusive, person-centered care models through presentations, resource-sharing, and consultation.
  • Promote smoother transitions of care and advocate for cultural responsiveness and accessibility in community healthcare settings.
  • Track outcomes related to resident self-advocacy, provider engagement, and care coordination improvements.

  • RequirementsValid New York State RN license.
  • 2+ years nursing experience, preferably with populations requiring complex care coordination.
  • Strong understanding of intellectual and developmental disabilities, or willingness to undergo specialized training.
  • Excellent communication skills with a demonstrated ability to educate diverse audiences.
  • Ability to work collaboratively with interdisciplinary teams and external stakeholders.
  • Passion for advocacy, equity, and empowering marginalized communities.
  • Experience working in residential or long-term care settings.
  • Background in public health, health education, or care transitions.
  • Familiarity with rural health systems and behavioral health integration.

Salary Range

  • $76-79k
