Relief Direct Support Professional - NANTUCKET - Community Connections : Job Details

Community Connections Inc. is looking for Relief Direct Support Professionals to join our Day Center team based on Nantucket MA, on a relief basis. In this role, you will support individuals with disabilities by implementing their Day Habilitation Service Plan (DHSP) and documenting their progress. You will help people become more engaged and reach their maximum potential in physical, emotional and social abilities. You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for a Developmental Specialist with a commitment to compassion, promoting accessibility and serving the community.What You'll Do as a Direct Support Professional:Implement Day Habilitation Service Plan (DHSP) objectives Document each person's progress towards DHSP objectivesAssist individuals physically as necessary Ensure individuals health, safety and general well being Provide and maintain community based habilitation/therapeutic experiences through social, recreational and volunteer opportunities, based on the individual's needs and wishesEnsure and safeguard individuals' human dignity, rights, and liberties What You'll Need as a Direct Support Professional:High School Diploma or General Education Degree (GED)1+ year's experience and/or training working with individuals with disabilitiesValid driver's license and satisfactory driving recordProven problem solving abilities Excellent written and verbal communication and interpersonal skillsDemonstrated ability to work independently Strong computer and word processing skillsWho You Are as a Direct Support Professional:An advocate for individuals with disabilitiesSomeone who wants their weekends freeSomeone who enjoys working as part of a teamSomeone who is willing to learnJob Type: Part Time, Relief
