Become the Rehabilitation Case Manager You Aspire to Be

  • Collaborate with an interdisciplinary team to guide treatment plans based on patient needs.
  • Coordinate with the team to establish tentative discharge plans and contingency strategies.
  • Participate in planning and executing positive patient discharge experiences.
  • Monitor patient experiences: quality, timeliness, appropriateness, and expectations.
  • Facilitate weekly team conferences and coordinate treatment plan adjustments.
  • Complete all case management documentation as required.
  • Stay up-to-date with regulations, company policies, and department operations.
  • Analyze case management reports and develop actionable plans.
  • Understand commercial contract levels, exclusions, and payor requirements.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and reduce ACTs.
  • Meet with patients and families within 24 hours of admission to establish communication.
  • Assess goals and complete documentation within 48 hours of admission.
  • Educate patients and families about rehabilitation services and the role of the Case Manager.
  • Assist patients in procuring resources to prevent discharge delays.
  • Ensure compliance with regulations regarding orthotics and prosthetics.
  • Make timely referrals and document to post-discharge providers.
  • Verify the accuracy of discharge and payor-related information.
  • Engage in the utilization review process: data collection and trend assessment.
  • Participate in the on-call case management schedule as needed.

Qualifications

  • Licensure or Certification:
    • Must independently complete assessments within the scope of your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling).
    • If licensure is needed in your discipline, an active license is mandatory.
    • Must meet eligibility for CCM® or ACM certification upon entry into this position or within two years.
    • CCM® or ACM certification is required or must be obtained within two years of entering the Case Manager II position.
  • Minimum Qualifications:
    • For Nursing, an Associate Degree in Nursing and RN licensure is required, with BSN preferred; a diploma is acceptable only if state's licensure requirement permits.
    • For other certified or licensed healthcare professionals, a minimum of a bachelor's degree is required, with a graduate degree preferred.
    • At least 2 years of rehabilitation experience is preferred.
