POSITION SUMMARY: We have an exciting opportunity to join our team as a Associate Transplant Coordinator. In this role, the successful candidate Supports the Transplant Institute through provision of administrative tasks such as answering patient phone calls, scheduling testing, and correspondence for patients and referring providers, and other administrative duties as assigned.

JOB RESPONSIBILITIES:

  • Performs any and all other administrative duties assigned.
  • Demonstrates knowledge of the organization's Service Standards and incorporates them into the performance of duties.
  • Runs EPIC reports of patients requiring follow up and blood work.
  • In collaboration with transplant coordinator, facilitate transition of care to Nephrology post-transplant.
  • Provides referring physicians with status reports about patients' progress in the transplant system via letters, phone calls, emails, or other forms of communication.
  • Provides patient education about the transplant and living donation process and what to expect from the transplant center to patients, living donors and families.
  • Independently answers clinical questions from patients, donors and referring providers.
  • Reviews and coordinates patient testing and tracks progress in EPIC checklist, synthesize results and reports to transplant coordinators, follows up with patients regarding missing testing.

MINIMUM QUALIFICATIONS: To qualify you must have a Bachelor degree or three years of transplant experience

PREFERRED QUALIFICATIONS: Bachelor degree preferred or three years of transplant experience.
