Responsibilities:

Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility opening this summer. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting.

  • Provide specialized wound and skin care treatments, ensuring timely, evidence-based interventions that support optimal healing outcomes.
  • Collaborate with physicians and interdisciplinary team members to develop, implement and evaluate resident care plans for wound and skin-related needs.
  • Monitor skin integrity and treatment progress, documenting findings and updating care plans as needed to prevent complications and promote healing.
  • Educate residents, caregivers and nursing staff on best practices for wound prevention and skin care maintenance.

Qualifications:

Education & Experience

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
  • 1 year of nursing experience required

Licenses & Certifications

  • RN - Current and valid Registered Nurse license to work within the state of California required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
