We are seeking a Manager - Home-Based Care Programs to:

  • Lead an inpatient department that provides acute care to patients in the comfort of their home.
  • Manage an interdisciplinary care team, including providers, clinicians and support staff, to ensure that patients receive exemplary inpatient care in their home.
  • Direct and evaluate the department's staff performance and scheduling.
  • Build collaborative relationships with internal and external partners in this market.
  • Strong candidates will demonstrate confidence in leading a multi-disciplinary team who works primarily off-site in patient's homes; excellent communication and people management skills; an innovative approach to leadership; experience in home health or hospice settings (preferred).

Qualifications

  • Bachelor's Degree in Nursing (BSN) Required
  • Master's Degree , Doctorate, or PhD in Nursing or health care related field Preferred

Work Experience

  • 3 years of clinical experience with knowledge of health care systems Required
  • Progressive experience in clinical program development and management Preferred

Licenses & Certifications

  • Wisconsin state RN licensure Upon Hire Required
  • CPR certification Upon Hire Required
  • Certification in area of specialty or eligibility for certification Preferred
